Shift:5x8 Day Shift ' 8:00 AM to 4:30 PM
Guaranteed Hours:40

Pay Details:
For Local Candidates pay rate: $55 to $60 /hr (If your permanent address is within 50 miles of the facility)

For Travelers/Stipend Candidates (If your permanent address is outside the 50-mile radius from the facility):
Blended Hourly Pay Rate (W2): $58.00
Weekly Pay Rate (W2): $2,320.00
Minimum Hourly W2 Pay Rate: $15.00
Minimum Hourly Stipend: $43.00
Min. W2 Weekly Gross (Taxable): $600.00
Weekly Stipend (Non-Taxable): $1,720.00
Blended Weekly Gross Pay Rate: $2,320.00

For Traveler 1099 Candidates:
Hourly Pay Rate (1099): $60.00
Weekly Pay Rate (1099): $2,400.00

Job Summary:

We're seeking a Community Health LPN to provide skilled, compassionate nursing care to patients in their homes. The LPN will assess patient and family needs, provide and document nursing interventions, coordinate care with healthcare professionals and community providers, and evaluate patient responses to treatment. This role requires strong clinical judgment, independence, critical thinking, and flexibility while managing a home health caseload with minimal supervision.

Requirements:
  • Active Massachusetts LPN License required
  • AHA BLS or American Red Cross BLS required
  • 1+ year acute nursing experience required; hospital-based preferred
  • Home Health experience required
  • Certified Home Health experience with OASIS proficiency highly preferred
  • Infusion Therapy experience preferred
