Exciting Primary Care Opportunity Bronx - NY

Join a highly respected non-profit organization dedicated to providing impactful healthcare services to underserved communities. This role offers an incredible opportunity for a compassionate Primary Care Provider (APP) to make a meaningful difference in the lives of youth at a transitional housing site in the Bronx.

Position Highlights:

• Schedule: Monday-Friday
• Hours: 9-5, or 8-4, some flexibility!
• Setting: Transitional Housing
• Position Type: Full-time, permanent

Key Responsibilities:

• Provides medical care to pediatric, adolescent and adult patients, takes relevant patient history, and identifies patient needs and makes diagnoses
• Observes, implements, and promotes the federal regulations as defined by HIPAA and Policies and Procedures
• Assists the Chief Medical Officer in the development and implementation of medical protocols and medical forms
• Works collaboratively with physicians and specialists for consultation and referral purposes

Qualifications:

• 2+ year of relevant experience as an Advanced Practice Provider
• Active NYS PA or FNP license.
• Board/DEA certified with access to Medicaid
• Passion for working with underserved or at-risk populations.
