Position Overview:
We are seeking a dedicatedNurse Practitioner (NP) or Physician Assistant (PA)to join ouroutpatient Family Medicine teamat anFQHC clinic near Patten, Maine. This role offers the opportunity to make a meaningful impact by serving the underserved while working in a collaborative and supportive primary care setting.
Key Responsibilities:
- Provide comprehensive outpatient primary care services to patients of all ages.
- Diagnose, treat, and manage acute and chronic conditions.
- Promote health and wellness through patient education, preventive care, and screenings.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated, high-quality care.
- Maintain accurate patient documentation within the EMR system.
- Participate in quality improvement and population health initiatives.

- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Maine (or eligibility for licensure).
- Family Practice certification or relevant clinical experience required.
- Experience in community health, primary care, or working with underserved populations preferred.
- Strong interpersonal, communication, and teamwork skills.
- Commitment to mission-driven, patient-centered care.
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Maine (or eligibility for licensure).
- Family Practice certification or relevant clinical experience required.
- Experience in community health, primary care, or working with underserved populations preferred.
- Strong interpersonal, communication, and teamwork skills.
- Commitment to mission-driven, patient-centered care.
