Position Overview:

We are seeking a dedicatedNurse Practitioner (NP) or Physician Assistant (PA)to join ouroutpatient Family Medicine teamat anFQHC clinic near Patten, Maine. Key Responsibilities:

• Provide comprehensive outpatient primary care services to patients of all ages.

• Diagnose, treat, and manage acute and chronic conditions.

• Promote health and wellness through patient education, preventive care, and screenings.

• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated, high-quality care.

• Maintain accurate patient documentation within the EMR system.

• Participate in quality improvement and population health initiatives.

Qualifications:

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Maine (or eligibility for licensure).

• Family Practice certification or relevant clinical experience required.

• Experience in community health, primary care, or working with underserved populations preferred.

• Strong interpersonal, communication, and teamwork skills.
