POSITION DUTIES: Serves as a staff pharmacist in a pharmacy supporting a medium to large hospital with a large volume of outpatient clinic prescriptions, a high diversity of patient care and a rapidly changing patient population. Pharmacist is required to work in all specialty areas; e.g., unit dose, outpatient, sterile products, during normal duty hours and on rotating shifts. 1. As a licensed pharmacist, independently performs pharmacy functions of drug selection, compounding and dispensing of a broad range of therapeutic agents in support of highly diversified clinical functions and hospital wards. Checks prescriptions carefully, applying a thorough knowledge of the dosage and incompatibilities of drugs prescribed. Compounds pharmaceutical preparations, applying a thorough knowledge of the physical and chemical properties and characteristics of substances used; the therapeutic actions; and the principles of chemical, biological and pharmacological sciences. Also applies a knowledge of therapeutic physical and chemical incompatibilities in evaluating dosages and permissible concentration of drugs and in anticipating, preventing, and treating undesirable chemical reactions resulting in insolubility, inactivation, and precipitations. Exercises professional judgment in generic substitution and dosage adaptation. Formulates a variety of complex intravenous additives and solutions and sterile irrigations. Prepares preoperative medication for anesthesia. Monitors laboratory values for possible drug lab interferences. Provides information and consultive advice to prescribing physician, PA, RN, practitioner, etc., on contraindications and side effects; suggests alternative medications to avoid incompatibilities, alleviate side effects, prevent antagonistic reactions, improve palatability, etc. Provides consultation to patient in selection of over-the-counter medications for treatment of minor illness, compatibility of personal diet and prescribed medication, use of aspirin or other over-the-counter medications with prescribed medications, etc. Properly labels, records, applies directions for use and appropriate cautions to products dispensed. Is personally responsible for the development and preparation of 'extemporaneous compounds in application to a wide variety of pharmaceutical needs. 2. As a senior pharmacist, provides technical guidance to lower graded pharmacists and pharmacy assistants in resolving problems of difficult, complex, or unusual nature. Conducts special studies in the use and cost of drugs and the feasibility of preparing compounds as opposed to purchasing the complete product and the proper location and time for formulating the compounds. Monitors and reviews the dispensing of narcotics and other controlled drugs. Provides continuing on-the-job training and occasional classroom instruction to other pharmacy personnel. Provides in-service training to medical staff. Performs other duties as assigned
