Guaranteed Hours: 40

Pay Details:

This position is open to travel candidates only.

For Travelers/Stipend Candidates (If your permanent address is outside the 50 mile radius from the facility):

Blended Hourly W2 Pay Rate: $54.00
Blended Weekly Gross Pay Rate: $2,160.00
Minimum Hourly W2 Pay Rate: $15.15
Hourly Stipend: $38.85
Min. W2 Weekly Gross (Taxable): $606.00
Weekly Stipend (Non-Taxable): $1,554.00

OR

For Traveler 1099 Candidates:
Pay Rate on 1099 to the Candidate: $56.00
Weekly 1099 Traveler Pay Rate:$2,240.00

Our client is seeking an experiencedLicensed Practical Nurse (LPN)to provide high-qualitycommunity and home health nursing care, with a strong focus on patient assessment, care coordination, health promotion, documentation, and independent clinical decision-making in the home setting.

Key Responsibilities:
  • Provide and document nursing care to sick and injured patients in their homes.
  • Promote, maintain, and restore patient health while supporting rehabilitation and illness prevention.
  • Coordinate care with healthcare providers, professionals, and community representatives.
  • Evaluate patients' and families' responses to nursing interventions.
  • Provide care independently while following established guidelines, protocols, and policies.
  • Utilize critical thinking, time management, communication, planning, delegation, and problem-solving skills.
  • Maintain accurate clinical records and communicate patient information verbally and in writing.
  • Participate in on-call responsibilities and maintain flexibility with work hours.

Qualifications:
  • Education:Nursing education/program completion 'Required
  • Experience:Minimum 1 year of acute nursing care 'Required; hospital-based experience preferred
  • Home Health experience ' Required
  • Certified Home Health experience, including OASIS proficiency ' Highly Preferred
  • Infusion Therapy experience ' Preferred
  • Experience with self-scheduling of caseload ' Preferred.
  • BLS ' Required
  • Active Massachusetts LPN License ' Required
