Job Summary

We are seeking an experienced LPN/LVN for a Long-Term Care (LTC) assignment in Claremont, NH. The ideal candidate will provide compassionate nursing care to residents while working closely with the healthcare team to maintain their safety, comfort, and well-being.

Key Responsibilities

• Provide direct nursing care to LTC residents according to care plans.
• Administer medications and treatments as ordered.
• Monitor residents' conditions and report changes to the appropriate healthcare provider.
• Perform assessments and maintain accurate clinical documentation.
• Assist with activities of daily living and promote resident independence.
• Communicate effectively with residents, families, and the healthcare team.
• Follow facility policies, infection-control procedures, and safety standards.

Requirements

• Active LPN/LVN license eligible to work in New Hampshire.
• Previous Long-Term Care (LTC/SNF) experience preferred.
• Current BLS certification.
• Strong communication and patient-care skills.
• Ability to work the scheduled evening shift.
• Updated resume and professional references.
