A health services network in New York City is currently seeking a licensed Registered Nurse (RN) to join their staff as their new Assessment Nurse. In this field role, the Assessment Nurse (RN) will be responsible for doing UAS assessments throughout Upper Manhattan.

Responsibilities:

The Assessment Nurse (RN) will be responsible for:

• Performing home visits to conduct functional, psychosocial and environmental safety assessments
• Determining eligibility of enrollees
• Establishing an individual plan of care that enables them to remain safely at home despite long-term care needs
• Ensuring that DOH UAS information is complete and accurate
• Conducting follow-up assessments to ensure the members are receiving the proper care and services.
• Performing other duties, as needed

Qualifications:

• 2+ years of related Nursing experience
• Graduate of an accredited School of Nursing
• NYS Registered Nurse (RN) license
• UAS background
• Solid assessment, clinical, and documentation skills
• Patient oriented
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized

Desired Skills:

• Bachelor's Degree in Nursing
• Bilingual (English and Cantonese / Mandarin or Spanish)
