Primary Care Physician opportunity in Rochester, NY

Wonderful full-time primary care role providing patient care in a post-acute care setting in a skilled nursing facility (SNF) located in the Rochester, New York, area. You will round 4-5 days a week with the flexibility to choose your rounding schedule.

Position highlights:

• Primary care rounding in skilled nursing facilities
• Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
• Collaborate with clinicians, facility leaders and others regarding care and services
• Provide oversight, guidance, and mentorship to our nurse practitioners
• Conduct clinical case reviews as needed and share tools, information, and guidelines as they relate to quality of care

• Minimum of 1 year primary care experience
• Post-acute care experience a plus
