• Monday - Friday 8 am - 5 pm schedule
• 15 - 18 patients per day
• Outpatient clinic setting
• Support midlevel providers with clinical guidance
• Comfortable with homeless and mental health patients required
• Both chronic and acute care management
• Hospital privileges required
