• Board Status: BC/BE in Family Medicine or Internal Medicine

• License: Any state license accepted (IHS facility)

• Certifications:

• BLS, ACLS, PALS

• TB clearance (PPD or chest X-ray if prior positive)

• 2 MMR vaccinations or immunity (not required if born before 1957)

• Hepatitis B series

• Seasonal influenza vaccine

• COVID vaccination

• Practice History: No lapse in clinical practice in the 6 months prior to start

• Built-in Admin Time: Wednesday mornings (clinic opens at noon)
