ROLE PRIMARY CARE
Schedule Monday - Friday
Shared call
Conduct thorough evaluations and assessments of patients, including taking medical histories, performing physical examinations & ordering diagnostic tests.
Develop and implement individualized treatment plans based on patient needs, medical history & diagnostic results.
Provide preventive care services, including routine check-ups, immunizations & health screenings.
Manage chronic conditions such as diabetes, hypertension & asthma, and provide ongoing monitoring and treatment.
Address acute medical issues, including infections, injuries & urgent care needs.
Educate patients and their families about health maintenance, disease prevention & lifestyle modifications.
Collaborate with other healthcare professionals, including specialists, nurses & support staff, to provide coordinated and comprehensive care.
Communicate effectively with patients and their families about diagnoses, treatment options, and care plans.
Participate in care coordination and case management to ensure seamless patient care.
Maintain accurate and up-to-date medical records in accordance with hospital/clinic policies and regulatory requirements.
Participate in quality improvement initiatives and clinical audits to enhance patient care and operational efficiency.
Stay current with medical advancements, best practices & guidelines in primary care.

Training: Completed an accredited residency in Internal Medicine, Family Medicine, or a related field.
BC/BE Family Medicine , will consider BC/BE Internal Medicine
