POSITION DUTIES: - Leads a multi-disciplinary team of individuals who collectively take responsibility for the ongoing care of patients. - Responsible for all provider credentialing and privileging completion. - Provide comprehensive outpatient care for beneficiaries ranging from newborn to geriatric age. - Coordinate and/or integrate care across all elements of the healthcare system. - Coordinate with facility and community resources to provide multi-disciplinary comprehensive care to all patients - Provide professional guidance to other health care providers as assigned. Performs other duties as assigned.
