Job Description
Job Description:
- Schedule is Monday through Friday 8:00 a.m. to 5:00 p.m., or until all patients are seen.
- Call is 1:7 (phone call only)
- Conventional Family Medicine practice treating chronic disease management; dressing changes; sutures; biopsies; pap smears; joint injection/aspiration
- Basic laboratory services are available and basic procedures such as skin tag and mole removal, suturing, and joint injections may be performed.
- Treating pediatrics to geriatrics
- Candidate will join one Nurse Practitioner
- Average patient census per day is 18-22
- Opportunities for teaching exist with St. Joseph Health System Family Medicine residents [clinical teaching and supervision].
- Must adhere to the Ethical and Religious Directives for Catholic Health Care Services.
- May be required to maintain active membership status on the medical staff of SJHS-SB and/or SJHS-Ply.
- EMR: Epic
Compensation and Benefits:
- Competitive salary range in line with MGMA standards.Provider may submit salary expectations with presentation.

- Health/dental/vision/disability/life insurance/retirement benefits
- 27 days off per year; 7 holidays; 5 days and $3,000 for CME
- $3,000 per year for professional journals, dues, and memberships
Submission Requirements:
- TRULY Board Eligible (within 5 years of residency) or Board Certified in Family Medicine (preferred) or Internal Medicine-REQUIRED
- IN license (or willing to obtain) -REQUIRED
- Active DEA (or willing to obtain)-REQUIRED
- BLS andACLS (or willing to obtain)-REQUIRED
- COVID Vaccination -REQUIRED
- Provider will be working with all ages - must have experience working with pediatrics-geriatrics(note experience with all ages in "Experience" field)-REQUIRED
- EPICEMRknowledge- HIGHLY PREFERRED
