Major Duties: SOCIAL WORKER (CLINICAL) - (GS-0185-12) - Perform bio-psychosocial assessments on behavioral health patients to determine a diagnosis. - Perform routine case management functions including telephonic monitoring of symptom severity and/or adherence to behavioral health treatment plans. - Provide targeted assessment and evaluation, including diagnostic impressions and/or functional status focused on the presenting problem. - Formulate behavioral health interventions. - Provide consultation services for a subset of patients who require ongoing monitoring and follow-up. - Provide counseling services using various evidence-based modalities as clinically indicated.
