Job Description

Well-established group with multiple locations in Charleston, SC seeking Psychiatric Nurse Practitioners to join their team. You'll work alongside a dedicated team within the community clinic. The position is full time and permanent, offering great benefits, PTO, sick time, vacation time and more. The ideal candidate would be committed to quality care and have a passion for working in a team environment.

You can expect to provide primary health care to individuals and their families and your assigned site and possibly at satellite sites as needed. You will conduct patient assessments, evaluate patients' mental and physical health, perform mental status exams, review medical history to diagnose and treat psychiatric disorders.

Additional responsibilities may include:

Develop treatment plans based on the patient's diagnosis, create comprehensive treatment plans that may include medications, therapy, lifestyle changes and referrals to other healthcare providers.

Prescribed medications to manage psychiatric symptoms such as antidepressants, antipsychotics, and mood stabilizers.

Provide individual, group, and therapy family therapy to help patients manage their symptoms and improve their quality of life.

Collaborate with other health care providers such as psychiatrists, psychologists, social workers, and primary care providers to ensure coordinated and comprehensive care.

Document all patient care, maintain accurate and up-to-date patient records, including assessments, treatment plans, progress notes, and medication orders.

Responsible for accurate, timely, and complete documentation in patient records in accordance with the group's policy.

Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each visit.

Requirements:

Minimum of two years private practice experience or one year experience in a community Health Center.

Current registration in the state of South Carolina as an advanced practice nurse.

Spanish bilingual preferred.

Certification as nurse practitioner from the American nurses credentialing center.

A Master of Science degree in nursing from a program accredited by the American Nurses Association of American Association of nurse practitioners.

Ability to work independently and within a team environment.

Strong organizational skills, problem solving skills, and analytical skills.

Able to exercise sound judgment and discretion in managing patient and employee issues.
