Job Description

A health services organization in California is currently seeking a new, licensed Registered Nurse (RN) to join their team in San Diego County as a Case Manager.In this role, the Case Manager (RN) will be responsible for providing ongoing support and expertise through comprehensive assessment, care planning, plan implementation and overall evaluation of individual patient needs.

About the Opportunity:
  • Start Date: ASAP
  • Assignment Length: Contract to permanent
  • Schedule:Monday to Friday
  • Hours: 8:30am to 5pm
  • Patient Population: Adults (ages 18+) dealing with Strokes, IVs, and PICC lines
  • Patient Volume: 10 to 15 per caseload
  • Call: None
  • EMR:PinPoint and UDS
Responsibilities:

The Case Manager (RN) will:
  • Completedepartmental orientation, initial and annual competencies
  • Assistwith departmental specific performance improvement initiatives collecting and reporting data
  • Consultother departmental staff to collaborate in patient care delivery, identify barriers to care and or discharge and develop solutions/resolution
  • Completedocumentation per workflow timeline and content requirements i
  • Coordinateweekly patient care team conferences to facilitate development, monitoring and refinement of treatment plan to achieve identified patient goals and outcomes
  • Communicateeffectively with nursing, therapy and other ancillary departments to ensure proper utilization
  • Participateas the facility representative for national conference calls and communicates new information to the facility'sCase Managers
  • Assistwith concurrent and retrospective utilization review activities including denials and appeals
  • Performintake assessment on patient within 24 to 72 hours of admission
  • Performfollow-up assessments per Case Management Plan and/or hospital policy.
  • Perform other duties, as assigned
Qualifications:
  • Bachelor's Degree
  • CA Registered Nurse (RN) license
  • Computer savvy
  • Microsoft Office Suite proficient (Excel, Outlook, PowerPoint, Word, etc.)
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
Desired Skills:
  • 2+ years of Case Management and/or Social Work experience in an Inpatient setting
  • Certification in one of their following: Certification in Case Management or Rehabilitation Nursing; Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC
  • Experience in an Acute / Rehabilitation Hospital setting
