Duties: - Perform independent, comprehensive telephone and clinical triaging in the primary care setting, supporting primary care providers in their treatment of patients with behavioral health conditions. - Monitor patient progress and symptom severity by conducting a structured telephonic interview and using validated tools. - Collaborate with patients to develop SMART self-management goals to assist patients with symptom improvement. - Assist clinic with patient and staff education and provides individual and small group training in order to increase referrals and promote understanding of the role of the behavioral health care facilitator. - Work collaboratively with the Nurse Case Manager to provide comprehensive, coordinated care Performs other duties as assigned.
