• Monday - Friday, 8am - 5pm schedule
• 12 - 15 patients per day
• 3 days field work, 2 days telehealth from home
• Hybrid role combining home health visits and telehealth
• Geriatric primary care focus
• Minimum 3 years adult primary care experience required
• Must be confident working independently
• Hospital privileges required
