POSITION DUTIES: Employee is a community health nurse responsible for providing primary health care and generalized public health nursing services in clinics, homes and schools and assists the community in planning and evaluating the local health delivery system. 1. Child and Youth Services (CYS) Health Consultant. a. Administer and interpret AR 608-10 and interim guidance on the health requirements for staff and participants, and sanitation of facilities and homes. Conduct monthly site assessment visits; participate on the Installation Evaluation Team for youth center, school age services, child development center, and family child care. b. Plan, coordinate, conduct and evaluate classes on medication administration, communicable diseases, blood borne pathogens, pediatric health conditions and treatments, and health promotion practices. c. Advise and develops action plans for the Family Advocacy Program Special Needs Accommodation Process (SNAP) and Case Review Committee on pediatric health and developmental concerns. d. Respond to requests for exception to policy situations for example: medications or immunizations. e. Advise on and report communicable disease clusters or outbreaks. Coordinate with civilian health care providers, hospitals and local public health departments. 2. Epidemiology program manager. a. Plan, conduct and evaluate epidemiology and disease surveillance programs. Develop objectives, standards, and policies to ensure the delivery of comprehensive epidemiology in accordance with medical, legal, and regulatory requirements. Implement surveillance policies and programs IAW DOD, Army, TRICARE, and Health Affairs guidance. b. Perform functions with significant independence for Epidemiology, Disease Epidemiology Reporting System (DRSi). Execute disease outbreak investigations and perform analysis to include: attack rates, incidence rate, prevalence, and relative risk, target groups, cause of outbreak based on the strength of the association, consistency of data, biological credibility, and use tools of descriptive and analytical epidemiology. c. Provide clinical services to eligible multi-service beneficiaries to include those at outlying military Reserve and National Guard training installations. Provide case management in consultation with the physician consultant. Order and interpret diagnostic and screening test results to confirm or rule out assessments. Tracks contact makes assessments and arranges testing to determine health status. Refers individual for treatment based on observation of symptoms and laboratory results. Perform case contact interviews. Prescribe treatment plans and prescribe medication when appropriate per established protocols for Sexually Transmitted Diseases, Tuberculosis Surveillance and HIV. Monitor for adverse reactions to medications and symptoms. Provide counsel to client, family, and/or contacts regarding risks, preventive measures, disease etiology, transmission, and course of disease as well as treatment measures for contagious conditions. Plan, coordinate, and implement educational programs for community and military groups on the nature, transmission, and control of Tuberculosis, Sexually Transmitted Infections and Diseases to include HIV infection, and diagnoses of public health significance. Renews prescriptions for medications during follow up visits. d. Communicate with installation, county and state public health departments and military regional medical centers manage and/or report data, facilitate or conduct epidemiologic investigations, or coordinate care within the region. Maintain liaison with Public Affairs Office, troop commanders, clinic personnel, and health authorities for the purpose of disseminating information and coordinating services to reduce the spread of communicable diseases. e. Assist Chief, PM to plan, coordinate and conduct mass immunization programs to include MILVAX and annual influenza programs. f. Ensure the reportable events and communicable disease prevention and control program is critically reviewed and monitored by an ongoing process improvement program. g. Utilize computerized word processing, data base and spreadsheets. Utilize a diverse set of automated systems, to include, the Composite Health Care System (CHCS I and/or II), Automated Data System (ADS), AHLTA, MEDPROS, Medical Occupational Data Systems (MODS), and Epi Info. 3. Health Promotion Coordinator. a. Serves as the Health Promotion Program Manager under Community Health Nursing Section of Preventive Medicine Service. Directs and coordinates work of health promotion staff to include, one or two licensed practical nurse. Responsible for overseeing and participation of MEDPROS screening, immunizations and laboratory requirements for Soldier Readiness in-processing. Responsible for coordination of installation scheduled Health Fairs and the Civilian Fitness Program. Independently develop, conduct and analyze needs assessments for program planning, assessing health status, identifying health risks, identifying resources, cost benefit analysis, and establishing priorities for health promotion and disease prevention activities within the Military community, in accordance with AR 600-63, Army Health Promotion. Develop and implement research and evidence based curricula and lesson plans for health education programs, classes, short courses, and training seminars. Provide health awareness activities to include monthly health observance information and newspaper articles, and health screening activities at community fairs and forums. Manage budget and procure supplies for the program activities. b. Establish outcome objectives for health promotion programs that enable comparison with Healthy People 2020/Healthy Communities national public health objectives. Develop written protocol and procedural guidance for implementation and evaluation of primary, secondary and tertiary prevention programs. Evaluate and interpret documents and data resources from federal, state and local governmental agencies to make recommendations for changes in health promotion programs. Coordinate efforts with Public Health Command (Provisional). c. Assess, recommend, and evaluate multi-disciplinary lifestyle management interventions for clients with pre-morbid health states to enhance productivity and efficiency of the work environment and medical readiness of multi-service military forces. Conduct screenings to assess health promotion and maintenance needs in individuals and aggregates; provide preventive services such as immunizations and counseling related to lifestyle health risks. Coordinate multi-disciplinary tobacco cessation group classes, prescribe and monitor pharmaceutical and behavioral therapies. Review laboratory values to evaluate health risk factors; counsel and/or refers patients with abnormal values for additional nutritional or medical evaluation. d. Outpatient Clinical Dietetics: Performs clinical dietetic activities outpatient nutritional care, group and individual outpatient education, health promotion, and performance improvement activities. Provides weight control counseling to all military and beneficiaries in accordance with AR 600-9. Assists the Chief, Preventive Medicine, and organizational Dietician in the accomplishment of departmental goals and objectives. e. Represent the Command, and Chief, Preventive Medicine on interagency and interdepartmental committees, boards, and meetings involving matters related to health promotion and disease prevention. 4. Public Health Preparedness Nurse Consultant. a. Participate on multi-disciplinary teams for Public Health Preparedness and emergency response to natural disasters and man-made threats to public health and safety. b. Interface with local military and civilian healthcare facilities, public health departments, public safety officials, reference laboratories, and veterinary providers. 5. Travel Medicine. a. Screen applicant for medical history, travel itinerary, and other travel-related factors to determine health risks. Screen immunizations and health records for overall compliance with routine immunization recommendations. b. Make recommendations for additional screening tests (for example, serologic titers), immunizations, chemoprophylaxis, personal protective measures, and other medical advice based on geographic location of travel. c. Refer screened applicant to Health Care Provider for immunizations, medications, and other screening tests as required. Provide advice on measures to reduce travel-related health risks. d. Obtain appropriate vaccines to be administered from the pharmacy and administer as appropriate. 6. Community Assessment and Coordinator with Lyster Army Health Clinic Services. Conduct community assessments including identifying needs, assets and priorities. Assesses individual, group, or community health status; estimate the services required for primary, secondary and tertiary prevention programs; and institute proactive interventions focused on reducing unnecessary health care utilization; assess the performance and outcomes of interventions. 7. Administration. a. The complexity of the above activities require substantial administrative work to document and implement required activities such as writing documents, developing and updating internal Standard Operational Procedures (SOP) relating to program operations annually, documenting and maintaining patient records who have received counseling or direct patient services, preparing reports and developing educational materials and tools. Develop performance work plans. Contribute to performance and evaluation report. Ensure compliance with the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and The Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule. b. Develop and maintain annual program document, monthly activity calendar, military unit health risk assessment profiles, monthly activity report, supply and equipment procurement actions, quality improvement program, and budget projections and requests. c. Sustain personal and clinic staff professional development by participating in professional meetings, continuing education courses, reading current professional journals and literature. Performs other duties as assigned.
