Job Description
A nonprofit services organization in New York City is currently seeking a new Case Manager to join their growing team in Queens.In this role, theCase Manager will be mainly responsible for providing case management services for a caseload of 10-15 clients and their family members and preparing comprehensive assessments and service plans.

The Case Manager will:
- Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals
- Conduct face to face contacts (home visits/office visits) as specified in the service plans
- Review the case management service plans and conduct reassessments on a quarterly basis to ensure that services are appropriate for the clients current needs
- Provide crisis intervention to clients and their families, as needed
- Maintain ongoing awareness of clients medical conditions so that services are appropriate to changing needs
- Arrange support and home healthcare services as necessary, making referrals to medical, home care, mental health and/or substance abuse services
- Ensure that clients pay the balance of their rent, telephone and utility bills
- Provide counseling and orientation regarding tenants rights and responsibilities to maintain and retain housing
- Assist clients in moving from hospitals, nursing homes or other locations into their apartments, and, provide transportation when necessary
- Maintain and complete required documentation including all contacts made with the client and any contact on their behalf
- Perform any other duties, as assigned
- 6+ months of related HRA / HASA experience
- Associate's and/orBachelor's Degree
- Individual, Family and Group Counseling experience
- Knowledge of at least one of the following service areas: Homeless-related issues, Criminal Justice, HIV/ AIDS, Substance Abuse, etc.
- Extensive knowledge of Case Management principles and practices
- Solid assessment and documentation skills
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
- Bilingual
- Experience working in Scattered Site programs
- Experience using AWARDs & CARES documentation system
- Customer service oriented
