Job Description

A nonprofit services organization in New York City is currently seeking a new Case Manager to join their growing team in Queens.In this role, theCase Manager will be mainly responsible for providing case management services for a caseload of 10-15 clients and their family members and preparing comprehensive assessments and service plans.

Responsibilities

The Case Manager will:
  • Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals
  • Conduct face to face contacts (home visits/office visits) as specified in the service plans
  • Review the case management service plans and conduct reassessments on a quarterly basis to ensure that services are appropriate for the clients current needs
  • Provide crisis intervention to clients and their families, as needed
  • Maintain ongoing awareness of clients medical conditions so that services are appropriate to changing needs
  • Arrange support and home healthcare services as necessary, making referrals to medical, home care, mental health and/or substance abuse services
  • Ensure that clients pay the balance of their rent, telephone and utility bills
  • Provide counseling and orientation regarding tenants rights and responsibilities to maintain and retain housing
  • Assist clients in moving from hospitals, nursing homes or other locations into their apartments, and, provide transportation when necessary
  • Maintain and complete required documentation including all contacts made with the client and any contact on their behalf
  • Perform any other duties, as assigned
Qualifications
  • 6+ months of related HRA / HASA experience
  • Associate's and/orBachelor's Degree
  • Individual, Family and Group Counseling experience
  • Knowledge of at least one of the following service areas: Homeless-related issues, Criminal Justice, HIV/ AIDS, Substance Abuse, etc.
  • Extensive knowledge of Case Management principles and practices
  • Solid assessment and documentation skills
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
Desired Skills
  • Bilingual
  • Experience working in Scattered Site programs
  • Experience using AWARDs & CARES documentation system
  • Customer service oriented
