Job Description

A social services organization located in Brooklyn is actively seeking an organized and team-oriented Case Manager to join their growing staff. In this role, the Case Manager ensures that each client is receiving appropriate and effective case management services.

Responsibilities

The Case Manager will:
  • Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals
  • Conduct face to face contacts (home visits/office visits) as specified in the service plan(s); with a minimum of one home visit per month and one contact per week
  • Provide assistance in accessing primary medical care
  • Review the case management service plans and conduct reassessments on a quarterly basis to ensure that services are appropriate for the clients current needs
  • Provide crisis intervention to clients and their families as needed
  • Maintain ongoing awareness of clients medical conditions so that services are appropriate to changing needs
  • Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services
  • Provide counseling and orientation regarding tenants rights and responsibilities to maintain and retain housing
Qualifications
  • Undergraduate degree
  • 6+ months- 2 years of field work / HRA/HASA direct work experience
  • Individual, family, and group counseling experience, particularly with HIV+/AIDS affected populations and/or substance abuse individuals
  • Knowledge of MS Word, PowerPoint, and other database programs
  • Knowledge of at least one of the following service areas: homeless-related issues, criminal justice, HIV/ AIDS, substance abuse
  • Extensive knowledge of case management principles and practices
  • Good initiative and learning ability of HRA-HASA Scattered Site Program Desk Guide and the HIV/AIDS Program, Office of Mental Health regulations
  • Responsible, organized, effective, and efficient
Desired skills
  • Experience using AWARDs & CAIRS documentation system 
  • Some customer service and job readiness experience
