• Must have active state license
• BC required
• Weekdays, Days, Weekend call
• Need a minimum of 500 cases in a year with case logs & with preferably 5 yrs of experience
• Average of 20 - 30 patients per day in clinic, 1 - 2 patients per week while on-call
• General ENT procedures including head & neck, ear & nasal surgery, laryngology, facial plastics etc
• Credentialing needed
• DEA needed
