Job Description

We are seeking a physician who is board certified or board eligible who has a passion to serve patients in all levels of post-acute care including LTAC, post-acute rehab, inpatient rehab and skilled nursing.

Responsibilities: 
  • Work alongside pharmacists on medication management
  • Complete Medicare required visits
  • Complete acute, follow up, and discharge visits as necessary
  • Implement an individualized plan of care for patients' goals
  • In collaboration with the IDT determine transitional care
  • Make recommendations on DME, home health services, or outpatient services after short stay
