Position Highlights:

• 34 patient contact hours with 6 administrative hours per week
• Support from a full Patient-Centered Medical Home team that includes clinical pharmacists, behavioral health clinicians, care managers, nurse practitioners, physician assistants, registered nurses, and medical assistants.
