Michigan Post-Acute care facility located on the coast of the Great Lakes is seeking a physician to join their team. This is a full-time opportunity rounding in skilled nursing facilities.

Perform comprehensive patient assessments, including medical histories and physical exams.  Develop, implement, and adjust individualized care plans based on patient needs.  Monitor progress and coordinate care to optimize outcomes.  Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach.  Educate patients and families on health management and preventive strategies.  Maintain accurate, timely medical records in compliance with regulations.
