The interdisciplinary team includes on-site social work, physical and occupational therapy, a registered dietician, a day center coordinator, home care, transportation, and more. - Build trusted relationships with participants and their families Requirements: - MD/DO with interest in geriatrics - BE/BC in IM, FM, or Geriatrics - Active CA license + DEA - Meets 42 CFR Part 410.20 - Minimum 1 year experience with the frail/elderly population
