• Monday - Friday schedule, 7:30 am - 5 pm with shared phone call
• Hospital-employed position with full administrative and clinical support
• Leadership role overseeing and mentoring primary care providers at assigned center
• Manageable patient panel with focus on older adult primary care
• On-site diagnostics including ultrasound, echocardiogram, stress tests, and EKG
• Lead interdisciplinary care team including medical assistants and care coordinators
• Board-certified geriatric medicine physician required; DEA required
• Outpatient-based role with strong clinical-operations culture and team support
