Position Highlights:

• Clinic-only practice along with outreach to senior living facilities
• Optometry, Chiropractic, Behavioral Health, and General Surgery on-site with an Ambulatory Surgery Center
• Works closely with Family Medicine Providers; 2 Physicians and 6 APPs
• $339,106base salary plus potentialbonuses, quality incentives, paid malpractice,and a comprehensive benefits package.
