OVERVIEW

Join an established neighborhood primary care/geriatric clinic, using a value based care model, to ensure patients are seen & heard year round preventing complications from multiple chronic conditions.

ROLE

' Avg. 14-17 patients a day

' Monday thru Friday, NO WEEKENDS

' Providers per clinic: Average 3-5 (Primary Care Physicians + NP's)

' Onsite Lab & Pharmacy

' Care team: Scribe, MA, Social Worker

PACKAGE

Salary range $240-$260K+ (commensurate with experience/ site)

Quality Bonus annually plus Promotion Track to Leadership

'Ability to obtain/maintain medical license in state of Missouri
'1+ years experience required
'Board Certified/Board Eligible Internal Medicine, Family Medicine or Geriatrics
