An opportunity is available at a community health clinic in Brooklyn, NY for a Family Nurse Practitioner (FNP) to deliver primary care to patients of all ages in an underserved community.

About the Opportunity:

• Shift: Flexible within clinic hours
• Schedule: Monday - Thursday 8:00 AM - 7:00 PM, Friday 8:00 AM - 5:00 PM
• Hours: Full-time
• Setting: Outpatient community health clinic

Responsibilities:

The Family Nurse Practitioner (FNP) will be responsible for:

• Performing comprehensive physical exams and health assessments
• Diagnosing and treating acute and chronic conditions
• Ordering and interpreting laboratory tests and diagnostic imaging
• Prescribing medications and managing treatment plans
• Providing wellness visits, screenings, and immunizations

Qualifications:

• Minimum 1 year of clinical experience
• Master's or Doctorate degree in Nursing (MSN or DNP)
• Active New York RN and Nurse Practitioner license
• Board Certification as a Family Nurse Practitioner (FNP)
• Experience with EMR systems, specifically Athena Health
• Medicare/Medicaid credentialing required
• Strong communication skills
• Ability to work collaboratively in a team-based environment
• Bilingual Spanish preferred
