MAJOR DUTIES AND RESPONSIBILITIES - Serve as the Primary Care Optimization (PCO) team leader providing comprehensive outpatient care to beneficiaries ranging from newborn to geriatric age presenting with complex and interdependent acute and chronic conditions. - Provide pre- and post-deployment health assessments as well as preventive health assessments (PHA). - Complete administrative paperwork and assessments required of active duty Primary Care Managers. - Advise other physicians, non-physician providers, and nursing and para-professional staff on the care of patients with urgent, emergent, and non-urgent medical problems. - Provide clinical and administrative leadership to the Patient Centered Medical Home (PCMH) team. - Serve as member of the professional medical staff following and modifying hospital plans and policies to meet accrediting agencies requirements. - Participate in local disaster training exercises as directed by the Chief of Medical Staff or MTF Commander; responds to real-world disaster situations. - Coordinate with local, state, and county medical emergency personnel to deliver primary care following the assigned facilitys Medical Contingency Response Plan (MCRP). - Participate in a rotating call schedule, averaging one week every six weeks, during which the member will hold the designated call phone for Active-Duty death notifications. Such notifications are infrequent and will occur only on rare occasions. Performs other duties as assigned.
