Role OverviewPosition Type: Temp-to-Perm, Family MedicinePractice Setting: Hospital-based Family Primary CareEMR: MeditechPatient Ages: 6 years through end of lifeTypical Patient Mix: Majority over age 45Pediatric Volume: ~40 patients (ages 617) out of 5,600 total censusModel: Longstanding Patient-Centered Medical Home (nearly 20 years)Schedule & Availability
  • Clinic Hours:
    • MondayWednesday, Friday: 7:00 AM7:30 PM
    • Saturday: 9:00 AM2:00 PM
  • Flexible scheduling within FTE requirements
  • Coverage Needed: Every day except Thursday
Work Details
  • Support Staff: Fully supported clinic team
  • Patients per Day: ~18 PPD
  • Coverage Need: Ongoing until transition to perm
