Utica, New York, Post-Acute Care Physician OpportunityClinical Role:

Thisfull-time primary carephysicianrole will include patient care rounding in apost-acute care settingin askilled nursing facility (SNF)located in theUtica, NY, area. You will round 5 days a week with the flexibility to choose your rounding schedule.
Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
Collaborate with clinicians, facility leaders and others regarding care and services
Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
Provide oversight, guidance, and mentorship to our nurse practitioners
Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care
Minimum of 1-year primary care experience
Post-acute care experience a plus, but not require
Hospitalists, internal medicine, family medicine, emergency medicine and related specialties are encouraged to apply
Active and unrestricted New York license or application in process
Active and unrestricted DEA/prescriptive authority or application in process
Expected compensation is $183,222 to $221,114 annually with no cap on productivity income potential.
