A healthcare network in Connecticut is actively seeking a licensed Nurse Practitioner (NP) to join their Primary Care team in Fairfield County. In this role, the Primary Care NP will be responsible for managing a patient panel with a broad array of patient needs.

About the Opportunity:

• Schedule: Monday to Friday
• Hours: 32-36 per week

Responsibilities

The Primary Care NP will be responsible for:

• Conducting a mix of acute, chronic, and well visits.
• Treating patients in-office or in testing centers as well as conducting occasional tele-health visits
• Continuous learning during weekly Clinical Rounds and through other modalities
• Ongoing collaboration with in-office teammates via daily huddles, as well as with virtual clinical teams
• Performing in-office procedures, as needed
• Handling other duites, as needed

Qualifications

• 2+ years of experience in a Primary Care role/setting
• Bachelor's and/or Master's Degree
• Completed an accredited Nurse Practitioner (NP) program with a national certification
• CT Nurse Practitioner (NP) license
• Clinical proficiency in evidence-based Primary Care
• Solid assessment, clinical, and documentation skills
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized
