Searching for a Geriatrician to join a team in the planning, implementation, and evaluation of care for the elderly through the PACE Program.

About the position:

• Flexible Schedules
• Completes initial, periodic, and annual physical examinations of the participants
• Evaluates participants for episodic visits, for acute illnesses, and follow-ups of hospitalizations in the clinic, at home, or in a nursing home
• Makes nursing home visits
• Use telehealth as needed and where appropriate
• Participates in the PACE hospital rounding system and the on-call schedule
• Evaluate the needs of the participants and develop treatment plans in concert with the other members of the Interdisciplinary team
• Market compensation with incentives plus benefits package
