As a Clinical Social Worker you will: - Collect bio-psychosocial information about patients using interview, case history, observation techniques, and assessment methods. - Provide individual and/or group psychotherapy. - Work with the multi-disciplinary team to develop disposition planning. - Function as a member of the multi-disciplinary treatment team, presenting internal reports and participating in patient evaluation and treatment planning. - Evaluate treatment effectiveness and make adjustments to patient's treatment plans as appropriate. - Utilize electronic health record systems to document and review patient data. - Performs other duties as assigned.
