This ACT facility specializes in community-based psychiatric care for individuals with severe and persistent mental illness through mobile outreach programs. The psychiatric nurse practitioner will provide specialized ACT team services for adults with severe and persistent mental illness who often experience frequent hospitalizations, homelessness, or treatment non-compliance. Clinical duties include conducting psychiatric evaluations and medication management in various community settings while collaborating with the multidisciplinary ACT team to deliver intensive mobile psychiatric services. The position requires full-time commitment working Monday through Friday during day shift hours with prescriptive authority to manage complex psychiatric conditions and substance use disorders in non-traditional healthcare environments.
