Nurse Practitioner - Cook County suburb- 10 minutes from downtown Chicago- Outpatient

·               Monday to Friday - 8a-5p- No nights, No weekends!

·                Visits: 20 min. regular visit/ 40 min. full diagnostic and new patient

·                Seeing 13-16 patients a day

·               Physicians care team: Scribe, MA, Social Worker

·                Equipment at clinic: Labs on site with a phlebotomist/ access to RubiconMD (access to specialist)

Will be supported by the Medical Director in a value based care model for preventative health, concentrating on adults- with a large patient base which is bilingual Spanish

Salary

·                Range for both is 105,000-135,000 (Experienced provider with bonus can make up to $155k)

 Required Qualifications

·                Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate

·               Bilingual Spanish strongly preferred

·                Ability to obtain and maintain Illinois license

·                1 year experience preferred plus national certification in at 1 of following specialties:

Family Nurse Practitioner

Adult-Gerontology Primary Care Nurse Practitioner

Adult Nurse Practitioner

Gerontological Nurse Practitioner

·                Active, non-probationary state Nurse Practitioner license

·                Active DEA license
