Job Description
Our client is seeking an experienced Licensed Practical Nurse (LPN) for a Long-Term Care assignment in Claremont, New Hampshire. The LPN will provide high-quality resident care in a rural LTC setting, working independently and collaboratively with nursing leadership and interdisciplinary teams. The ideal candidate will have strong clinical judgment, prior LTC experience, and the ability to manage a full scope of nursing responsibilities in a small-facility environment.
Key Responsibilities:
' Monitor residents for changes in physical and mental condition and report significant findings promptly
' Administer medications, treatments, oxygen therapy, and enteral feedings per physician orders and facility protocols
' Perform wound care, including care involving wound vac systems
' Document medications, treatments, and resident care accurately in electronic MAR/TAR systems
' Receive, transcribe, and implement physician orders
' Supervise LNAs and delegate care based on scope of practice and competency
' Assist direct-care staff with nursing duties as needed
' Participate in physician rounds and communicate resident updates
' Complete admissions, discharges, and transfers with proper documentation
' Ensure infection control standards are followed and maintain a safe, clean unit environment
' Review and manage new orders, precautions, medications, and care plans
' Communicate effectively with residents, families, physicians, and nursing leadership
' Manage controlled substances, including shift-to-shift counts per regulations
' Assist with orientation and training of nursing staff
' Maintain responsibility for assigned unit during shift under supervision of Nursing Leadership
Qualifications:
' Education: Graduate of an accredited LPN program
' Experience: Minimum 3 years of recent LPN experience (within the last 6 months required)
' Long-Term Care experience required
' IV certification required (or willingness to obtain during onboarding)
' Travel experience highly preferred
' Rural or small-facility experience highly preferred
' Must be willing to float within areas of competency
' Active LPN license in New Hampshire (or compact eligible)
' Strong clinical assessment and documentation skills required
Important Notes:
' Assignment is in a rural location approximately 2 hours from Boston, MA
' Limited housing and public transportation available
' Candidates must have reliable personal transportation
' Strong independence and adaptability required for rural LTC setting
