• Monday - Friday 8:30am - 5:30pm
• 15 patients per day with longer visits for continuity
• Clinic setting
• Geriatrics focus with patients 55 years and up
• Must be PACE program trained provider
• Hospital privileges required
• BLS certification required
• 2 years NP experience with geriatric patients required
