- Assess and provide intervention for individuals with a wide range of chronic medical conditions, and behavioral health issues affecting medical conditions to include assessment and disposition of beneficiaries with acute and chronic risk. - Consult with primary care providers to select, approve, and document treatment priorities. - Educate clinic staff on the principles of primary care behavioral health via trainings and briefings. - Conduct outpatient group classes, stress management, relaxation training, assertive/social skills training, diabetes management, chronic pain, and brief related interventions. - Assess acute mental and behavioral medicine conditions in beneficiaries. - Provide individual and group treatment, using various evidence-based modalities to include problem focused interventions, cognitive-behavioral therapy, and psychoeducational classes. - Develop, and implement, professional development activities, coordinates departmental training on PCBH related topics. - Performs other duties as assigned
