- Assess acute, non-acute, and chronic clinical and psychological diagnosis. - Perform new patient intake/screenings to include medical history, physical examination, laboratory, and diagnostic studies. - Implement treatment plans and education for chronic conditions. - Collaborate with physicians regarding patient management. - Develop and/or conduct in-service education programs for other health care professionals. - Provides counseling to patients and their families regarding illness prevention and health maintenance. - Performs other duties as assigned.
