    • Average Number of Cases Per Day: 15
    • Documentation System/EMR: Epic
    • Number of Exam Rooms: 1
    • Patient Age Demographic: Adolescent, Child, Neonate, Adult, Geriatric
    • Procedures Required: Fluoroscopy, CT, US
    • Interventional Procedures Required: Hysterosalpingogram, Lumbar Puncture, Sentinal Node Injection, Myelogram
    • Board Certified - REQUIRED
    • BLS/ACLS Certified - REQUIRED
    • Active OH License & DEA - REQUIRED
    • Provider MUST have clean files to be considered
    • Close to OH - Preferred
