WHY YOU'LL LOVE THIS JOB

A multitude of healthcare companies claim they are revolutionizing the

healthcare industry, and leading the way to a better future of healthcare.

With all of those claims, it can be a challenge to distinguish who really

is doing things differently. If you are a Physician seeking a chance to

truly improve the way the healthcare system supports our most fragile

patients, this is the opportunity for you. Our patient population consists

of patients who are affected by multiple chronic illnesses. The combination

of these illnesses, and their individual and compounding treatment needs,

make navigating the healthcare system a particular challenge. For these

patients, having the full scope of their healthcare needs managed

longitudinally by a multi-disciplinary team gives patients a central

advocate, and helps prevent unnecessary hospital admissions for a

population that frequently feels they have nowhere else to turn. As a

Physician on this team, you will travel to the patients homes. In some

cases, you will travel to Skilled Nursing Facilities or Long-Term Care, in

other cases Active Retirement Communities, and in some cases independent

homes. This is a team that rallies around our patients. You can understand

our motivations by what inspires us, by what we celebrate. Patient

satisfaction ratings are important, yes. But more than that - patients

whose health is restored after years of struggle, patients who are able to

live life more fully and comfortably, and whose healthcare needs are met

without months in the hospital - these are the successes that motivate our

team. It takes a special kind of person to have a heart for this job, and

these patients. And, not everyone who has the heart for it can actually

perform well in the role. It takes a highly-skilled Physician who is

confident in his or her ability to make the right decision in a crucial

moment. You must feel comfortable making decisions autonomously while in

the home, but must also know when the patient truly does need additional

care that cannot be provided in the home setting. This job is not easy. It

pushes you to perform your best every single day, and calls on you when the

challenge is too difficult for others. There are hard moments. You have

on-call shifts. You are the person that the patient knows they can always

rely upon. And, in return, you have great reward. You get to work at the

top of your license. You collaborate with the upper echelon of healthcare

providers and clinicians. You change lives for the patients who need you

most. You are able to take the time you need to fully understand the root

causes of your patients symptoms, and identify the factors limiting their

ability to recover. You have a voice alongside those truly leading change

in the industry. And, you are fully employed, with steady income that is

not dependent upon patient quotas. You have opportunities to earn

significant bonuses, you have generous PTO, and much, much more. Are you up

for the challenge? Apply today! JOB DUTIES AND RESPONSIBILITIES
