Full- and part-time positions are available depending on your schedule and availability. We ask that candidates be board certified or board eligible in family medicine (FM), internal medicine (IM) or geriatrics. Previous post-acute experience in a skilled nursing facility or assisted living center is a plus but not required. 

This role performs and coordinates work with the patient, family or support members, inpatient case management team, and interdisciplinary care team members during post-acute admission for appropriate utilization of services, length of stay and safe discharge plan. The role communicates regularly on post-acute care of specific patients, as well as population level care and performance.

As medical director and part of the leadership team, you will be responsible for providing quality care services for the patients, diagnosing them, and administering medications. Work with patients with a short-term illness or those patients recovered from medical conditions that require ongoing treatment and medications. Monitor patients' medical histories, scheduling follow-up checkups, as well as other procedures. You should have excellent communication and organizational skills. You will manage patient caseload by performing evaluations and implementing therapeutic interventions through ADL training and exercise, identify and refer critically ill patients to local emergency department, examine patients with emergency criteria and determine the conduct and hospitalization, and maintain confidentiality of all residents, facility and/or physician relate information. You will complete process of ICD-9 coding, claims submission, claims follow-up, payment postings and adjustments.

Competitive Compensation, with an estimated base salary range of $176,000 to $242,000 annually with the opportunity to earn incentives.
