Details For this Opportunity:
-100% outpatient medicine with EPIC EMR
-Limited patient volume of highly complex patients 55 yrs. and older
-Probably see 6-10 patients per day
-Provide comprehensive care to vulnerable and comorbid patients of the highest complexity.
- All patients seen in a clinical setting with transportation provided if necessary
- Team of physician and APP cover a patient panel of approximately 150
- Mon-Fri 8am-5pm with call center taking all initial calls
-Salaried position starting at $285,000
-Health/dental/vision at no charge including family
-26 days PTO
