Join a nationally recognized 'Patient Centered Medical Home' providing acute, preventive & chronic disease care, care management & individualized treatment plans customized to meet the needs of each patient.
Working collaboratively as a member of the medical staff, the PCP will play a critical role in managing the medical needs of patients. The physician establishes preventive measures, diagnoses, and treats illnesses and disorders in a competent manner that is consistent with medical practice and protocols.
Perform physical examinations and preventive health measures within prescribed guidelines. Interpret, and evaluate diagnostic tests to identify and assess patients' clinical problems and health care needs. Record physical findings, and formulates plan and prognosis, based on patient's condition. Provide written home-going instructions. Collaborate with Physician and other health professionals to prepare comprehensive patient care plan as necessary
Prescribe or recommend medications or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. Refer patients for consultation or to specialized health resources for treatment, when necessary.
Maintain medical records (Using Electronic Medical Record software). Educate and coach advanced practitioners on best nursing practices. Communicate with and counsel family members on treatment options material changes in status.
