Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA)

Position Overview:

As a Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA), you will play a vital role in providing primary healthcare services to individuals and families across the lifespan. You will collaborate with a team of dedicated healthcare professionals to deliver holistic and patient-centered care, emphasizing preventive care, health promotion, and disease management.

Responsibilities:

  1. Patient Assessment: Conduct comprehensive health assessments, including medical history, physical examinations, and diagnostic tests, to formulate accurate diagnoses and treatment plans.

  2. Diagnosis and Treatment: Develop and implement evidence-based treatment plans, prescribe medications, order and interpret diagnostic tests, and provide therapeutic interventions to manage acute and chronic health conditions.

  3. Health Education: Educate patients and their families about health maintenance, disease prevention, and treatment options, ensuring that they understand their healthcare plans and are empowered to make informed decisions.

  4. Chronic Disease Management: Manage and monitor patients with chronic diseases such as diabetes, hypertension, and heart disease, offering continuous support and adjustments to treatment plans as needed.

  5. Preventive Care: Administer vaccinations, perform routine screenings, and counsel patients on lifestyle modifications to promote optimal health and prevent illness.

  6. Collaboration: Collaborate with other healthcare providers, including physicians, nurses, and specialists, to ensure coordinated care and referrals as necessary.

Qualifications:

  1. Master's degree in Nursing (NP) or Physician Assistant Studies (PA-C).
  2. Current state licensure as a Nurse Practitioner (NP) or Physician Assistant (PA).
  3. Prior experience in family medicine or primary care is preferred.
