Position Overview

The Psychiatric Mental Health Nurse Practitioner, under the direction of the Medical Director, works closely with PACE Physicians and the IDT to provide comprehensive psychiatric care to our participants.

 Key Responsibilities

- Provide psychiatric consultation, evaluation, counseling, and treatment

- Perform comprehensive psychiatric assessments (in-person or telehealth) and develop care plans

- Collaborate with physicians and IDT on discharge planning

- Refer participants to psychiatrists as needed

- Oversee care of participants in nursing homes with regular visits

- Act as a consultant to the IDT, communicating participant changes and coordinating 24-hour care

 Requirements

- Masters degree in nursing with a Psychiatric/Mental Health emphasis

- Certified as a Psychiatric Mental Health Nurse Practitioner

- Active Psychiatric Mental Health Nurse Practitioner State License (CA)

- State CDS Certification, DEA License, and NPI license
